Evidence belongs with the requirement it supports.
A workflow lesson from food-safety documents.

Preserve the connection.
A document is useful when the people reviewing it can find the question it answers. Keep the record, its source, its review status and the relevant requirement connected. Avoid replacing the original evidence with an untraceable summary.
Krolda provides a useful product context for this problem: food-safety documents arrive from different suppliers and need to be related to the evidence a reviewer is seeking. This is a workflow example, not certification advice.
Make missing information visible.
Identify records that are absent, out of date or waiting for review. Give each exception an owner and a next step. A list of uploaded files does not by itself explain whether a required control has usable evidence.
Keep a history of changes so a correction does not erase the earlier record. People need to understand what changed and which version was used in a decision.
Design for the review.
Test whether a reviewer can move from a requirement to the supporting record and back. Include the source and the approval context needed to evaluate it.
The same pattern is useful beyond food safety: preserve the relationship between a claim, the evidence behind it and the person responsible for reviewing it. The requirements themselves must be assessed by the relevant qualified specialists.
